Medial cruroplasty is the dedicated surgical procedure for correcting skin ptosis on the upper inner thighs. This area is particularly susceptible to the effects of gravity, ageing, massive weight loss, and repeated fluctuations in weight. The medial (internal) technique used by Dr. Fathi places the incision within the inguinal crease — the natural fold connecting the groin to the buttock. The resulting scar is concealed beneath the line of standard underwear or swimwear.

Clinical evaluation of ptosis severity, standing skin assessment, incision marking, blood tests, anaesthesia workup.
ECG, full blood panel, mandatory anaesthetist consultation.
Morning of surgery, fasting from midnight.
Intubation, continuous monitoring throughout.
Gynaecological position to expose the bilateral inguinal fold.
Bilateral excision of excess skin along the inguinal crease, with lymphatic preservation.
Deep fascial sutures (Scarpa's fascia) + intradermal skin sutures.
Applied before recovery from anaesthesia.
2–4 hours; then transfer to ward room.
The following morning after Dr. Fathi's ward round, with prescriptions and written post-operative instructions.
Pulling sensation in the groin area, significant thigh swelling; rest with legs slightly elevated.
First dressing change; progressive mobilisation; return to sedentary work from Day 7–10.
Compressive shorts garment mandatory; bruising resolving; short walks permitted.
Gentle swimming and brisk walking permitted; scar still pink-red in colour.
Progressive scar fading in the inguinal fold, improved thigh firmness visible.

The scar follows the natural inguinal crease from the groin toward the buttock. When correctly positioned by Dr. Fathi, it remains beneath the line of standard swimwear and underwear. The final discretion of the scar depends on individual healing and the style of swimwear chosen.
The medial technique addresses primarily the upper third of the inner thigh. For more extensive ptosis extending toward the knee, a vertical technique may be necessary. Dr. Fathi will assess during the consultation which technical variant is most appropriate.
Yes. Dr. Fathi regularly combines these two procedures for post-bariatric patients. Combining procedures reduces the total number of anaesthesias and the overall recovery period.
Gentle walking is permitted from the first week. Brisk walking and swimming resume from 4 to 6 weeks. Lower-body strength training and high-impact exercise may resume from 8 to 12 weeks, subject to Dr. Fathi's approval.
Lymphoedema is a rare but recognised complication. Dr. Fathi minimises this risk through meticulous anatomical dissection with deliberate preservation of lymphatic structures. Should persistent swelling occur, manual lymphatic drainage therapy is prescribed.
For international patients, Dr. Fathi recommends a minimum stay of 7 to 10 days in Agadir to allow post-operative assessment and first dressing change before flying.
If prior liposuction has aggravated or revealed underlying skin laxity, inner thigh lift surgery may be indicated. A minimum interval of 6 to 12 months after the liposuction is required.
